Hospital Prescription Error Leads to Fatal Overdose

A man died from a morphine overdose just two days after being mistakenly prescribed the drug upon his discharge from Wrexham Maelor Hospital. The Public Services Ombudsman for Wales highlighted a series of failures by hospital staff, calling the incident an “extremely serious injustice”. The man, referred to as Mr P, had been treated for alcohol withdrawal and was incorrectly given morphine to take home, despite not having used it prior to his admission.

The ombudsman noted that proper checks were not conducted by the medical and pharmacy teams, which could have prevented this tragic error. Mr P’s widow expressed her devastation, stating that the prescription felt like sending her husband home with a “loaded gun”. The report emphasized that he should have been warned about the risks associated with opioid use, including the potential for fatal overdose.

Although it remains unclear if the hospital’s prescription directly caused his death, the ombudsman concluded that the lack of appropriate advice significantly heightened the risk of an accidental overdose. In response, Betsi Cadwaladr health board has issued an apology and committed to reviewing its practices to prevent future occurrences.

The ombudsman has recommended compensating Mr P’s family and ensuring that lessons are learned from this incident. This case raises critical questions about patient safety and the protocols surrounding opioid prescriptions in hospitals, highlighting the need for stringent checks to protect vulnerable patients.
