Investigation Reveals Engine Fault in Royal Navy Helicopter Tragedy

An investigation into the death of Lt Rhodri Leyshon during a Royal Navy helicopter exercise has uncovered a critical engine fault. The inquiry determined that a diaphragm failure in the helicopter’s engine led to a catastrophic loss of power. This incident occurred during a night-flying exercise off the coast of Dorset, resulting in the aircraft ditching into the sea.

The report highlights that the crew mistakenly believed two of the helicopter’s engines had failed, prompting them to shut them down. This decision ultimately caused the remaining engine to overspeed and fail as well, leading to the tragic crash. Lt Leyshon was unable to escape and was pronounced dead after being recovered from the water.

The implications of this incident extend beyond the immediate tragedy. The Defence Safety Authority has issued 44 recommendations aimed at preventing similar occurrences in the future.
