Inquest Reveals Treasury’s Role in Worker’s Tragic Death

An inquest has concluded that the Treasury ‘materially contributed’ to the death of Chloe Moffat, a 26-year-old personal assistant, by failing to follow its own disciplinary policies. The inquest revealed that Moffat was not informed she would not lose her job if allegations against her were proven, leading to her distress during a meeting with managers.

Moffat’s family expressed their belief that the Treasury neglected its duty of care, leaving her feeling isolated and frightened about her career. The coroner highlighted that had the proper procedures been followed, Moffat would have received a letter outlining the situation and the likely penalty, which would have been a written warning rather than dismissal.

This case raises significant concerns about workplace mental health and the handling of disciplinary processes within government departments. The coroner’s findings may prompt a review of how such procedures are conducted, aiming to safeguard employee welfare and prevent similar tragedies in the future.

The Treasury has acknowledged the coroner’s concerns and is considering recommendations for improving guidance and support for staff. The family hopes that meaningful changes will emerge from this tragedy, ensuring that no other family endures a similar loss.
