Surge in ‘Never Events’ Raises Concerns in Welsh Healthcare

The number of ‘never events’ at Betsi Cadwaladr University Health Board, Wales’ largest health board, has more than doubled in a year, raising serious concerns about patient safety. These incidents, which are preventable and should not occur if safety protocols are followed, increased from five to eleven in the latest reporting period. Examples include operating on the wrong body part or leaving surgical instruments inside patients.

This alarming rise highlights systemic issues within the health board, which has been under scrutiny since being placed in special measures in February 2023. The Welsh government intervened due to ongoing concerns about emergency care performance and cancer treatment waiting times. The health board’s leadership has acknowledged the public’s concerns and is committed to implementing changes to prevent future occurrences.

Investigations into these events focus on identifying failures in processes rather than assigning blame to individuals, aiming for meaningful improvements in patient safety. The board has set a goal of achieving zero never events by 2026-27, emphasizing the importance of learning from past mistakes to enhance healthcare delivery.

As the health board prepares to discuss these findings at its annual general meeting, the implications for patient trust and safety in the Welsh healthcare system are significant. The commitment to improvement will be closely monitored by both the public and health authorities moving forward.
