An inquiry into the tragic death of Royal Navy pilot Lieutenant Rhodri Leyshon has revealed a series of compounded errors that led to the fatal helicopter crash in the English Channel. The incident occurred during a training exercise with HMS Queen Elizabeth, where a malfunction in one of the Merlin Mk4’s engines triggered a catastrophic sequence of events. The crew misinterpreted a warning, leading the commander to switch off two functioning engines without proper confirmation, resulting in a loss of power and a fatal plunge into the sea.

The inquiry found that the crash should have been survivable, highlighting critical failures in both training and equipment. Investigators noted that the crew’s lack of recent experience and inadequate preparation for deck landings contributed to the tragedy. Furthermore, the faulty engine was linked to a design change by the manufacturer, which had weakened a crucial rubber component.

Rescue efforts were also hampered by equipment failures, delaying the recovery of Lieutenant Leyshon, who was not found until nearly two hours after the crash. The inquiry has made 44 recommendations aimed at improving safety protocols, including better maintenance practices, enhanced cockpit warnings, and more rigorous pilot training.

This incident serves as a stark reminder of the vulnerabilities within military training operations and the importance of adhering to safety protocols. The recommendations from the inquiry could lead to significant changes in how the Royal Navy conducts training exercises, potentially preventing future tragedies.
